It is very important to assign a different identifier for each element, except for the “radio buttons”. When using the “radio buttons” (the groups where just one of different items can be the selected one) it is critical that the identifier is the same for all buttons pertaining to the same group (it is the “on” value that differentiate this kind of form elements).
